Upon submission of the application documents, the applicant shall pay a non-refundable application fee as stipulated below under the “Fees” section of this page or as may be prescribed by the Board.It should be noted: If any of the details contained in the Application Form should change after the document has been signed and submitted, the applicant will be obliged to submit an updated application form within 30 days, otherwise, the Licence will automatically become null and void.The completed application form(s) together with the relevant documents indicated in the checklist or as listed under the “Required Documents” section of this page, should be submitted in person to the Pharmacy & Poisons Board, Registrar office, or via post.
